Proxy Expression Form
To download the “Proxy Expression Form,” click here. On this form, you are to indicate the church’s initial
expression for each Committee Report recommendation, and validate the form by
obtaining the signatures of the pastor (if you are the pastor serving as the
proxy, you may sign the form) and church clerk. Please remember, as the selected proxy representative you
are required to make your first expression according to the desires of the
local church you represent. After the first expression, you are at liberty to
change the expression if concerns are addressed during discussions or if it is necessary to come to
agreement. Subsequent expressions will be made using electronic “voting”
stations at the Assembly venue. On
each item being presented, you will have the option to select Yes, No, Abstain,
or Submit.
This form will not substitute opportunities for
dialogue and discussion; it simply expedites the process for receiving the
initial expressions. This form must be received by Proxy registration check-in by 11:30 a.m. EDT on Tuesday,
August 12 to insure that your first expression is processed correctly.
